Dr. Sniff requests that you please read these instructions prior to your visit.

Your First Appointment

You must arrive 20 minutes prior to your appointment time in order to complete the registration process. If you are able to print and complete new patient forms from the website prior to arrival, this will expedite the process. Please be prepared to provide a current list of all medications. Additionally, if you wish to submit a claim for out of network benefits to your insurance company, please bring the necessary forms to be completed.
Also, please bring the following:

  • Patient's insurance card
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records

Insurance and Payment Information

Dr. Sniff currently accepts Blue Cross/Blue Shield, Cigna, and is considered an out of network provider for other plans. We will assist in completing forms for reimbursement if a patient has out of network benefits.

Payment options include: Cash, Visa, Mastercard, American Express and Discover. We do not accept checks. Payment is due at the time of service, prior to being seen by Dr. Sniff.

Appointment/Cancellation Policy: There is a fee for canceling or rescheduling an appointment less than 24 hours in advance. The first occurrence is charged at a rate of half the appointment fee. All occurrences thereafter are charged the full fee. This fee is due prior to the next appointment.

Privacy Practices

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
